Agency-Specific Requirements
The federal regulations have been adopted by other agencies:
- American Cancer Society
- American Heart Association
- California Institute for Regenerative Medicine (CIRM)
- UC Discovery Grants
- UCOP Special Programs: University AIDS, California Breast Cancer, Tobacco Related Diseases
For these agencies, please complete the federal PI Certification form and the federal Disclosure of Financial Interest forms (as necessary) and submit the required form(s) with proposals for grants or contracts from these entities.
Additional Requirements or Exceptions
The American Cancer Society and the American Heart Association are exempt from disclosure under California law. Accordingly, Form 700U does not need to be completed for those two agencies.
The California Institute for Regenerative Medicine (CIRM) is a government sponsor. Accordingly, Form 700U does not need to be completed for this agency.
For UC Discovery Grants, see the separate COI web page about those grants. In addition to the PI Certification form, Form 700U is required from the PI.
For UCOP Special Programs, only three of the UCOP Special Programs require the PI Certification form: University AIDS, California Breast Cancer, and Tobacco Related Diseases. For all UCOP Special Programs, California Form 700-U is not required.
