How to Transfer a Grant to Another Institution

At the institution the investigator is leaving, a letter to the funding agency's Grants Management Specialist should be written by the Principal Investigator containing the grant number and all appropriate information regarding the grant transfer. Concurring signatures of the Chairperson, Dean of the school, and the Institutional Official should be appended to this letter.

The Grants Management Specialist will provide instructions for the completion of the transfer including the submission information needed by the investigator for the new institution.

  1. If the grant is from the NIH, a Relinquishing Statement, Form PHS 3734, is to be completed by the PI and should include signatures of the UCSF Accounting Officer and the Institutional Official. Send this form to the Accounting Office for signature first with a note to forward it on to C&G for signature and forwarding to the agency.
  2. If the grant is from the NIH, the Final Invention Statement, Form HHS 568, is to be completed and signed by the PI. This should be sent to C&G for countersignature and forwarding to NIH.
  3. The institution the investigator is transferring to will prepare a "change of grantee institution" proposal on the PHS 398 in accordance with the guidance from the Grants Management Specialist.
